Prior to participating in Whiteland Middle School during the current season, each player must have a completed IHSAA physical signed by a physician. The form is required to be dated after April 1, of the current calendar year. 

The Form must be submitted to the Middle School Athletic Director Taylor Treesh (ttreesh@cpcsc.k12.in.us) league prior to the first official day of practice.

Remember to submit all 6 pages of the completed (signed by a physician) form;
